How to register
We are happy to register new patients who reside within the practice area (see location map). Please contact the practice by telephone or come into the surgery where our receptionist will advise.
You may be required to make an appointment with the nurse.
If you are on regular medication, you will be offered an appointment to see the doctor.
You may choose the service of any of the doctors in the practice.
New Patient’s Procedure for Registering at the Practice
All new patients wishing to register at the practice must be dealt with in the following way
- New patients must come into the surgery and complete registration forms before being given appointments to register.
- Receptionist must check forms to make sure all relevant sections are completed.
- Receptionist must initial top of forms to indicate they have been checked and file forms in new patient file at front desk.
- New patients can then be given the appropriate registration appointments.
- Children under 5 are dealt with by the health visitor and don’t require doctor or nurses appointments
- Children between 5 and 14 and registered as part of a family with an ongoing condition - appointment with the doctor and the nurse.
- Children between 5 and 14 and registering as part of a family with no ongoing conditions - appointment with the nurse only.
- Children 14 and over must see a GP and a nurse to register.
- All adults must see a GP and a nurse.
